NEW PATIENT PAPERWORK
The “New Patient Package” can be emailed to you prior to your arrival at the clinic or it can be filled out in the office before your appointment. If you would prefer to fill your package out while in the clinic, please arrive 15 minutes early to keep your appointment on schedule.
YOUR HISTORY
Our doctors will look over your paperwork and ask you detailed questions about your health and what you are experiencing. Your detailed health history assists our doctors with your clinical diagnosis.
EXAMINATION
Using tests and assessments, our doctors will perform a physical exam to help confirm your diagnosis and determine the best possible avenues of care for you.
REPORT OF FINDINGS/
INFORMED CONSENT
After your history and examination is completed, our doctors will provide a diagnosis for you and suggest a care plan for you. They will also provide an informed consent to treatment which will outline important information about your care and allow you the opportunity to consent to care. Informed consent also addresses any concerns, questions, and gives assurances pertaining to your care.
TREATMENT
If your diagnosis aligns with the scope of chiropractic care, you will receive your first treatment at Orillia Chiropractic. It is our goal to provide you with treatment on the day of your examination.
